My name is Mandy Kerns and I am the Implant Treatment Coordinator here at Morgantown Dental Group. I am looking for patient(s) that have been through the implant procedure process to give testimonials for our website. In order to participate all I need is a brief written statement about why you first inquired about dental implants, what about our office/doctor made you choose to move forward, and finally how having dental implants has changed your day-to-day life.

I will be taking your photo in our office, and have you sign a quick release giving us permission to use your statement and photo for our website.

Patients who choose to participate in this project will receive a $50.00 gift card for our office, as well I will be providing a copy of the photos that I have taken for the website to you at no charge.
